Our aim is simple - to help you settle back into your home and ensure you're cared for and remain as independent as possible whilst you recover, and in the future. The Home & Well service gives you the confidence to return home safe in the knowledge that help and advice is available 24/7.

First 6 weeks completely FREE

Spending even a short period of time in hospital can leave you feeling anxious and worried about how you will cope when you get home. You may not want to rely on relatives for support and some may live too far for this to be practical. Most can't be available 24/7.

Our Home & Well service makes the transition from hospital to home easier and the first six weeks are completely free of charge. 

We understand that time is crucial to get the right support quickly in place when returning home from hospital. That’s why our services are tailored to help you and can normally be arranged within a matter of days.

Personal alarms
24/7 Personal Alarm Service

To know that someone is available 24/7 should you feel unwell, have a fall or simply need reassurance can be a real weight off your shoulders. 

Our personal alarm service means that we’re here at a touch of a button, whenever you need us, allowing you to recover worry free at your own home.

24/7 Medical Support Line

Put your mind at rest whenever you have a concern about your health or wellbeing by speaking to one of our qualified nurses.

They’re happy to talk to you 24/7 about any concern, no matter how big or small.

Medication Information Service

Our pharmacists are on hand to answer your questions and explain more about your pills and prescriptions.

They’ll help you understand why you have been prescribed them, how they interact with each other, and any possible side effects.

Hospital discharge services
Home & Well Check Calls

A regular chat on the phone can make a big difference when you’re living alone, especially if you’re worried or not feeling 100%.

We’ll call once a week to see how you are and listen to any concerns, and if you choose, we’ll make sure your family is kept fully up-to-date.

How to order Home & Well

To order the Home & Well service, call us on 0800 121 4521. The service is only available to order by phone.

First six weeks
We offer the first six weeks of our Home from Hospital service completely free of charge. This includes up to four ten minute calls from a Home and Well Check advisor. 

If you need the Personal Alarm Service urgently we can usually send it within two days for you to connect yourself.

After six weeks
We'll be in touch to see how you’re doing, and if you wish to continue. If you do, the following fees will apply:

  • A one-off discounted payment for the registration and set up fee: £35 
  • Monitoring cost for the 24/7 Personal Alarm Service, 24/7 Medical Support Line and Medication Information Service: £17 per month
  • If you would like to continue receiving a Wellbeing Check call: £84 per month for 20 minute weekly call

Safe home assessment
Safe Home Assessment

Our trained assessors can visit your home to ensure that it’s safe and suitable, and that all your care and support needs are met. You’ll receive a comprehensive safe home assessment report with recommendations.

A safe and simple way to store a door key so help can get to you in an emergency.
£65.99 with the key safe fitted free if installed alongside your personal alarm service

Case management service
Case Management

A dedicated Case Manager will provide personalised, practical and co-ordinated advice to help you navigate the health and social care system.

A weekly phone call by a designated member of our friendly helpline team can make a big difference to someone living alone, especially if you’re worried, feeling a bit low or can’t get out and about as often as you’d like.

Returning home from hospital

Who this service can help

You may benefit from our Home from Hospital service if you are:

  • Leaving hospital or due to have an operation and may return needing support or with mobility difficulties.
  • At risk of re-admission to hospital.
  • Worried about how you will cope when you get home from hospital.
  • Prescribed new medication, or struggling to understand your prescriptions.
  • Suffered a fall in the past, or are worried that your stay in hospital has left you a little unsteady on your feet.

Our service can also help if you have:

  • Epilepsy or diabetes.
  • A mobility disability or health condition that makes you at risk of falling.
  • Concerns about your health and wellbeing.
  • Friends or family who worry about you.

† Price excludes VAT which will be added at 20% where the customer is not eligible for zero rating. To be eligible for the Home & Well service, you must have been discharged from hospital six weeks prior to point of enquiry. Home & Well service also includes returning home from respite care.
